Daily Home Self Assessment Checklist
APS Staff/Student Daily Home Self Assessment Checklist:
Families/Students and staff are required to complete a self assessment every day before school. Do not come to school if any of the following are found during your daily self assessment.
- Check temperature
100.0 or higher - Do not report to school
- Symptom check
- Cough
- Sore throat
- Shortness of breath
- Chills
- Fatigue
- Muscle or body aches
- Headache
- Loss of taste or smell
- Nausea or vomiting
- Diarrhea
- Nasal congestion if any of the above symptoms are included
If you answered yes to any of the above do not attend school and contact your medical care provider for further advice.
